ADVERTISEMENT FOR BIDS
PROPOSALS ARE REQUESTED FROM GENERAL CONTRACTORS BY THE STATE OF FLORIDA, DEPARTMENT OF CORRECTIONS, FOR THE CONSTRUCTION OF:

PROJECT NAME & LOCATION: POLK CORRECTIONAL INSTITUTION ELEVATED WATER STORAGE TANK-REFURBISHMENT (EWST)
FOR: REFURBISHMENT OF THE ELEVATED WATER STORAGE TANK AT POLK CORRECTIONAL INSTITUTION
PERFORMANCE BOND AND LABOR AND MATERIAL PAYMENT BOND: If the construction contract award amount is $100,000.00 or less, a Performance Bond and a Labor And Material Payment Bond are not required.
PUBLIC ENTITY CRIME INFORMATION STATEMENT: A person or affiliate who has been placed on the convicted vendor list following a conviction for a public entity crime may not submit a bid on a contract to provide any goods or services to a public entity, may not submit a bid on a contract with a public entity for the construction or repair of a public building or public work, may not submit bids on leases of real property to a public entity, may not be awarded or perform work as a contractor, supplier, subcontractor, or consultant under a contract with any public entity, and may not transact business with any public entity in excess of the threshold amount provided in Section 287.017, F.S., for Category Two for a period of 36 months from the date of being placed on the convicted vendor list.
PREQUALIFICATION: Each bidder whose field is governed by Chapters 399, 489 and 633, Florida Statutes, for licensure or certification must submit prequalification data of their eligibility to submit proposals five (5) calendar days prior to the bid opening date if not previously qualified by the Department for the current biennium (July 1 through June 30) of odd numbered years. After the bid opening the low bidder must qualify in accordance with Rule 60D-5.004, F.A.C. A copy of the rule requirements is included in the Instruction To Bidders under Article B-2 Bidder Qualification Requirements and Procedures.
SEALED BIDS WILL BE RECEIVED, PUBLICLY OPENED, AND READ ALOUD ON:

Drawings and specifications may be purchased from the Architect/Engineer; hard copies $55.00 per set or electronically (a pdf sent via email) for no charge.
PRE-BID CONFERENCE: There will be no pre-bid conference for this project. However, Contractors are STRONGLY ENCOURAGED to visit the site on Wednesday, January 13, 2010, 9:00 a.m. (EST), to inspect the site and familiarize themselves with the project conditions. Contractors must contact: Mr. Ken Smith, Maintenance Supervisor at (863)984-2273, no later than Monday, January 11, 2010, to obtain clearance to visit the site, which is located at the Polk Correctional Institution, 10800 Evans Road, Polk City, Florida 33868-6925.
CONTRACT AWARD: Bid Tabulation and Notice of Award Recommendation will be sent to all bidders by Facsimile (FAX), Return Receipt Required. In accordance with Article B-20 of the Instructions to Bidders, Bid Protests, Points of Entry, if no protest is filed, a contract will be awarded by the Secretary, Department of Corrections. THE RIGHT IS RESERVED TO REJECT ANY OR ALL BIDS.
