NOTICE OF CHANGE

Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 48 No. 154, August 9, 2022 issue of the Florida Administrative Register.

Form DFS-N1-1732 is amended as follows:

The introductory paragraph is amended to read: As used in this application, “Division” refers to the Division of Funeral, Cemetery, and Consumer Services and “Board” refers to the Board of Funeral, Cemetery, and Consumer Services. Unless specifically indicated otherwise, all questions and requests for data in this application relate to the applicant. Where the answer is YES or NO, check the correct answer. Each of the related forms referred to on this application is incorporated by reference in Rule 69K-1.001, F.A.C.

Section 9(A1) is amended to read: (A1) Do you have a 2-year or 4-year college degree (e.g., a degree from a junior college, a community college, or 4-year college or university)?

YES   NO

Application and license fees are not refundable.

 

The instruction sentence to Section 9(A2) is amended to read: (A2) Check whichever of the following is applicable to you:

 

Section 13(e) is amended to read as follows: (e) Do you understand that, as part of this application, you must submit your fingerprints for a criminal background check?         YES   NO

Fingerprints must be submitted through the Department’s vender via https://www.identogo.com.

 

 

Form DFS-N1-1756 is amended as follows:

Section 9A is amended to read: A. If this application is approved, will the centralized embalming facility at all times contain all of the equipment and meet all of the requirements that a preparation room located in a funeral establishment is required to contain and meet?    YES    NO          

(see section 497.385(2)(a), F.S. and Rule 69K-21.003, F.A.C.)

 

Section 9B is amended to add parenthesis around number 5 for consistency.

 

Section 9H is amended to remove unnecessary language and will read: H. Do you understand that each principal of the applicant must submit their fingerprints for a criminal background check?
