NOTICE IS HEREBY GIVEN that on December 09, 2009, the Department of Financial Services, Division of Agent and Agency Services, received a petition for waiver filed by Liberty Life Insurance Company. The petition seeks a 30-day waiver from Rule 69B-162.011, F.A.C., in order to have more time to revise its systems before being required to use the forms adopted in Rule 69B-162.011, F.A.C.
