    NOTICE IS HEREBY GIVEN that on December 19, 2016, the Board of Optometry, received a petition for variance or waiver filed by George J. Brown, OD, from subsection 64B13-11.004(2), F.A.C., regarding the failure of the delinquent licensee to apply for active or inactive status during the biennium in which the license becomes delinquent and the license being rendered null and void without further action by the Board or the Department of Health.
