64B18-12.008 Registration Fee for Dispensing Practitioners.
A podiatric physician who dispenses medicinal drugs for human consumption for a fee or remuneration of any kind, whether direct or indirect, must register with the Board using the Dispensing Practitioner Registration form DH-MQA 1141 (revised 12/08), hereby adopted and incorporated by reference, that can be obtained from the Board of Podiatric Medicines website at http://www.doh.state.fl.us/mqa/podiatry/index.html, and pay a fee of $100.00 at the time of such registration and upon each renewal of licensure.
