NOTICE OF CHANGE

Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 38, No. 63, November 1, 2012 issue of the Florida Administrative Register.

With reference to Form DH-MQA 1251, the title of the form has been inserted in the rule text. On page four of the Form under Application Checklist, the Department corrected the required fee from $113.00 to $108.00.
