NOTICE IS HEREBY GIVEN that on February 11, 2015, the Board of Medicine received a petition for waiver or variance filed by Jean Otrakji, M.D., from Rules 64B8-1.007 and 64B8-4.009, F.A.C., with regard to the requirement for submission of documentation of medical education directly from Petitioner’s medical school and a waiver of the requirement for verification of Petitioner’s postgraduate training.
