64B12-16.004: Termination of Apprenticeship

The Board of Opticianry hereby gives notice of the issuance of an Order Denying Petition for Variance or Waiver regarding Jasmine Cox, filed on February 14, 2018. The Notice of Petition for Variance and Waiver was published in Volume 43, No. 237, of the December 11, 2017 issue of the Florida Administrative Register. No comments were received on the Petition. The Board considered the Petition at a duly-noticed public meeting on January 9, 2018. The Petitioner requested a variance or waiver from Rule 64B12-16.004, F.A.C., with regard to the requirement that an apprentice’s sponsor submit a termination of sponsorship form no more than thirty days after the termination date. At its meeting, the Board found that the application of the rule to the Petitioner would not violate the principles of fairness and therefore voted to deny the petition.
