Lake Butler Hospital Hand Surgery Center emergency service exemption
The Agency for Health Care Administration has received an application for an emergency service exemption from Lake Butler Hospital Hand Surgery Center located at 850 E Main St, Lake Butler, FL 32054 pursuant to Section 395.1041 (3), Florida Statutes and Rule 59A-3.255, Florida Administrative Code. The hospital is requesting an emergency service exemption for General Surgery and Plastic Surgery. Comments received within 15 days of publication will be considered by the Agency prior to making a determination of exemption status.
