NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 43 No. 7, January 11, 2017 issue of the Florida Administrative Register.
Incorporated form DH 1274 (effective 7/2016) has been revised to remove a question inadvertently left in the application relating to application of a health care fraud statute, repealed effective July 1, 2016, to candidates or applicants for initial licensure enrolled in an educational or training program on or before July 1, 2009. (See ch. 2016-230, § 15, Laws of Fla.).
The remainder of the proposed rule remains unchanged.
