NOTICE IS HEREBY GIVEN that on February 14, 2017, the Board of Opticianry received a petition for Variance or Waiver filed by Lisette E. Cabrera, on February 14, 2017. Petitioner seeks a variance or waiver of subsection 64B12-16.004(1), F.A.C., to extend the time frame for completing her apprenticeship. The Petitioner further states that she would suffer hardship due to an incomplete form filed by her sponsor. Except for good cause shown, motions for leave to intervene must be filed within 21 days after publication of this notice.
