NOTICE IS HEREBY GIVEN that on January 29, 2014, the Department of Health, Bureau of Radiation Control, received a petition for City Labs, Inc. The petitioner is seeking a waiver from subsection 64E-5.217(1), F.A.C., which provides the requirements for a performance bond for any applicant or licensee who is not exempt by the provisions of this subpart.
