NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 43, No. 217, November 8, 2017, issue of the Florida Administrative Register, and updated with a notice of change published in Vol. 44, No. 09, January 12, 2018, issue of the Florida Administrative Register.
Summaries of Substantive Changes to Amended Forms DBPR TA-1 and TA-2 are as follows:
61-19.003 – Amended Form TA-1:
The Effective Date of the Form was changed from “December 2017” to “January 2018” to reflect amended changes in the Form.
Eleventh unnumbered page, “Moral Character Affidavit”: Pursuant to Section 468.803(8), Florida Statutes, each application for licensure must be accompanied by at least five “good moral character” affidavits. Pursuant to ss. 92.50(1) and (2), F.S., “…affidavits…required or authorized under the laws of this state…” may be taken or administered by or before any notary public or justice of the peace. Therefore, the oath and/or affirmation statement contained in the “Moral Character Affidavit” of Form DBPR TA-1 will be removed and an area for notarization has been added to the form.
61-19.003(10) – Amended Form TA-2:

61-19.003 Application Requirements.
Each owner and operator of a talent agency wishing to obtain a license and renewal thereafter shall submit a completed application on Form DBPR TA-1, Application for Licensure as a Talent Agency, effective January 2018 December 2017, incorporated herein by reference, http://www.flrules.org/Gateway/reference.asp?No=Ref-08814, which can be obtained at www.myfloridalicense.com or by contacting the Department of Business and Professional Regulation, 2601 Blair Stone Road, Tallahassee, Florida 32399, together with the application fee as specified in Rule 61-19.005. In addition to the application form, each owner and operator shall submit the following information and documents:

(10) In the event the applicant for or holder of a talent agency license wishes to change or transfer ownership or operation other than that designated in the license, the applicant or licensee must complete and submit to the Department Form DBPR TA-2, Application for Change of Owner or Operator, effective January 2018 December 2017, incorporated herein by reference, http://www.flrules.org/Gateway/reference.asp?No=Ref-08815, which can be obtained at www.myfloridalicense.com or by contacting the Department of Business and Professional Regulation, 2601 Blair Stone Road, Tallahassee, Florida 32399, together with the application fee as specified in Rule 61-19.005.
