NOTICE OF CHANGE

Notice is hereby given that the following change has been made to the proposed rule to modify the application form number DH-MQA 1180, “Apprentice Optician Application”, and incorporated by referenced into the rule, in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 38, No. 80, of the October 29, 2013, issue of the Florida Administrative Register. The change is in response to concerns from the Joint Administrative Committee, in a letter dated December 5, 2012.

Form DH-MQA 1180, “Apprentice Optician Application”, shall be changed as follows:

One Page One, Section I of the application, the requirement for the applicant to provide the place of birth will be removed.

On Page One, Section I of the application, a statement will be added stating that providing an email address is optional. Also a statement will be added stating that providing an email address will become public record under Florida Law.
