61-35.011Cosmetology Departmental Forms

PURPOSE AND EFFECT: The proposed rulemaking seeks to amend Existing Rule 61-35.011, F.A.C., in order to amend DBPR application forms COSMO 1, COSMO 4-A and COSMO 4-B, and include submission of a Certificate of Completion. These amendments will ensure consistency with a recent amendment to 61G5-29.004, F.A.C., which now requires applicants to submit a copy of their Certificate of Completion with their application which is given by their training program, and to clarify outdated information within the instructions.
