NOTICE OF CHANGE

Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 41 No. 224, November 18, 2015 issue of the Florida Administrative Register.

The Department published form DH-MQA 1067, Application for School Psychology Licensure. Based on comments from the Joint Administrative Procedures Committee, the Department made the following changes:

 

1. The Department added section 456.0635, Florida Statutes, as a law implemented.

 

2. On page 10, item number 6, the Department removed language referencing an affirmation by the applicant. The changes are as follows:

 

From:

I affirm that I have completed the medical errors education required by section 456.013(7), Florida Statutes.

 

To:

I have completed the medical errors education required by section 456.013(7), Florida Statutes.

 

3. On page 14, item number 12, the Department corrected section “775.0083,” Florida Statutes, to read “775.083,” Florida Statutes.

 

On page 14, item number 12, the Department also removed the following language:

“…and I declare under penalty of perjury that my answers and all statements made by me herein are true and correct.”

 

4. On page 20, item number 5, the Department changed the word “Affidavit” to “Statement of Supervisor.”
