REQUEST FOR PROPOSAL
South Florida Community Care Network, LLC (SFCCN) is issuing a Request for Proposal (RFP) for After Hours and Nurse Helpline Services on March 11, 2016. A copy of the RFP may be obtained by contacting Donna Steinberg at (954)622-3225 or dsteinberg@sfccn.org.
Interested vendors may participate in a Question & Answer conference call on March 22, 2016, 10:30 a.m., ET by dialing 1(877)248-3057, ID: 2070369495.
The Receipt of Acknowledgment Form must be received by SFCCN no later than 5:00 p.m., ET, March 25, 2016.
Written follow-up questions must be received by SFCCN no later than March 29, 2016, 2:00 p.m. ET.
Responses to the RFP must be received by SFCCN no later than 12:00 Noon, ET, April 1, 2016, by sending proposals to the following:

All responses will be analyzed on the basis of the criteria set forth in the Request for Proposal.
The evaluation of the proposals received will be conducted from April 4-12, 2016. Selected vendors will be required to present an oral presentation on April 20, 2016 at SFCCN. SFCCN requests that vendors hold this date for on-site presentation.
The expected award date of the RFP is April 28, 2016.
SFCCN reserves the right to accept or reject, in whole or in part and for any reason whatsoever, any and all responses to the Request for Proposal and to waive any formalities in the response process.
