65C-13.025: Initial Licensing Procedures

NOTICE IS HEREBY GIVEN that on March 8, 2013, the Department of Children and Families, received a petition for waiver of Rule 65C-13.025, Florida Administrative Code, assigned Case No. 13-010W. paragraph 65C-13.025(3)(a), F.A.C., states the applicant shall provide the agency with proof of the following, if applicable; his or her current marriage license or most recent divorce decree as applicable, documentation of legal residency, driver’s licenses, auto insurance coverage, financial capability and income, child support verification, and pet vaccinations.
