    NOTICE OF CHANGE

    Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 39, No. 237, December 9, 2013 issue of the Florida Administrative Register.

    The change is in response to comments submitted by the Joint Administrative Procedures Committee in a letter dated October 25, 2013. The changes are as follows:

    The rule is to now read:

     

    64B14-4.115 Requirements for Renewal of Prosthetic or Orthotic Residency or Internship.

    To be eligible for a one (1) year renewal, the applicant must submit a renewal application to the department no earlier than 150 days prior to the expiration of the active residency or internship. The renewal application must be received by the Department while the initial intern or resident registration is still active and valid. The application titled “Internship/Residency Registration Renewal” is form DH-MQA-1268, 2/2014, herein incorporated by reference and available at: https://www.flrules.org/Gateway/reference.asp?No=Ref-______, and the Department’s website: http://www.doh.state.fl.us.us/mqa/OrthPros/index.html. The registration renewal fee is set in Rule 64B14-2.006, F.A.C., and must be included with the completed application.

    Changes to the Application are as follows:

    The form designation is now “DH-MQA 1268, 2/2014”

    All of No. 7. ID BADGE has been deleted from page one of the Application Instructions.

    An “E-mail Notification” paragraph has been added to page one under 2.d. of the Internship/Residency Registration Renewal.

    On No. 3 of the Internship/Registration Renewal Birth Place has been deleted.
