NOTICE IS HEREBY GIVEN that Department of Children and Families has received the petition for declaratory statement from Jacqueline Halyard, d/b/a Star Achievers Learning Center. The petition seeks the agency’s opinion as to the applicability of rule 65C-22.010(2)(e)2.a. Florida Administrative Code, and the Department’s January 31, 2017, Final Order of Dismissal, issued in Case No. 16-113CF, Rendition No. DCF-17-008-FO, as it applies to the petitioner.

In particular petitioner seeks a declaration whether the Department is required to amend its inspection reports to delete the violations recited in the January and May 2016 Warning Letters issued to Star Achievers and then subsequently withdrawn by the Department, as reflected in the Final Order of Dismissal.
