Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 37, No. 49, December 9, 2011 issue of the Florida Administrative Register.
The change is in response to written comments submitted by the staff of the Joint Administrative Procedures Committee. The changes are as follows:
1. Subsection (1) of the rule shall now read as follows:
(1) All persons applying for licensure as an electrologist shall submit a signed application to the Executive Director of the Council on forms provided by the Council and approved and incorporated herein by reference by the Board as Form DH-MQA 1164, 3/13 8/09, Electrologist Application, which can be accessed through www.doh.state.fl.us/mqa. The initial application must be accompanied by the application fee, as set forth in Rule 64B8-51.007, F.A.C.
2. In incorporated Form DH-MQA 1164 (3/13), the questions asked pursuant to Section 456.0635, F.S., have been updated to reflect the 2012 changes to that statute.
3. 456.0635, F.S. will be added to the Law Implemented.
