Project Location: Camp Blanding Joint Training Center, Starke, FL
Statement of Work: Remove 3 existing 36” culverts and headwalls, clearing and removing all trees, brush, sediment and debris as necessary to provide unobstructed entrance and exit to new box culverts. Furnish and install 50 linear feet of single 6’ wide x 4’ high precast concrete box culvert complete with appropriate wing walls at the upstream entrance and downstream exit. Box culvert shall be constructed to DOT index standards as indicated on the plans. Furnish and install 18” thick bed of rip rap with filter fabric underlayment extending out 15’ from the downstream edge of the box culverts and spanning the overall width of the wing walls.
Roadway to be finished with 12” stabilized sub base, extending 4’ beyond edge of roadway to provide stabilized shoulders. Surface of roadway to consist of 6” layer of recycled asphalt, approximately 150’ in length. One row of sod shall be installed along all paved surfaces. Other disturbed areas shall be regraded and treated with grass seed, fertilizer and mulch to FDOT grassing standards.
Furnish and install all necessary mechanisms, including but not limited to silt fence and floating turbidity barriers, to comply with the Storm water Pollution Prevention Plan indicated on the plans.
Provide Alternate Bid for concrete pavement driving surface over top of box culverts per Concrete Pavement Alternate details.
Estimated Construction Cost: $420,000.00
