REQUEST FOR STATEMENTS OF QUALIFICATIONS for GENERAL CONTRACTING WORK
The Sarasota County Public Hospital Board of Sarasota, Sarasota Memorial Hospital, Sarasota County, Florida (“the Hospital”) is accepting statements of qualifications from General Contracting Firms. The scope of work may include pre-construction phase services such as cost estimating, value engineering, critical path method scheduling, constructability reviews and cost control, in addition to presenting a phasing approach for a new, two-story expansion to the existing Sarasota Memorial Health Care Center at University Parkway building, located at 5350 University Parkway, Sarasota FL, 34243-5800. Level one will be design of a new PET-CT Suite and support service areas and an Interventional Radiology Suite and support service areas. Level two will be air-conditioned grey-level finished shell space. Both levels will connect and allow access to the existing facility. There will be significant connectivity, and access to the existing facility will need to remain active during all phases of the construction with a multi-phase approach with restricted contractor parking. Services required may include construction management and design assist for civil, mechanical, electrical, and plumbing, fire protection, structural and roadway, working in tandem with consulting design teams and the owner.
Firms interested in being considered as candidates are required to submit five bound statements of qualifications that include at least the following data, to be organized in the following order:
- A copy of Florida construction licensure and corporate registration certificates.
- Completed AIA Document A305 Contractor’s Qualification Statement, latest edition.
- Proof of general, automobile and workers’ compensation liability insurance coverage.
- A separate statement as to whether the firm is a certified Minority Business Enterprise. If your firm claims MBE, WMBE status, a copy of your firm’s current, valid MBE, WMBE certificate is required as part of your qualification statement submittal.
- A list of at least five client references consisting of name, title, address, telephone number and project name(s) for each project specified.
- Resumes of key personnel that would be used on this project to include relevant agency having jurisdiction experience.
- Past construction management experience with new multi-level construction expansions to existing facilities while remaining operational.
- Construction management / building experience within the County of Sarasota, FL.
- Past construction management experience with PET – CT, and Interventional Radiology specific projects and renovations as they relate to a combination of enabling and tertiary projects, including roadway access. Include projects requiring a multi-phased approach.
- Location of the firm’s main office.

The Hospital reserves the right to reject any or all submittals at any time during this process.
- The basis for selecting candidates includes, but is not limited to: submissions on time and complete, the firm’s experience with local regulatory agency having jurisdiction, consideration of directly related project experience, qualifications of proposed team, ability to respond, and project approach.
- The Hospital reserves the right to request additional information beyond the data set forth above.

- Submittals received after this deadline will remain unopened and available for pick up.
- Only Thomas Perigo shall be contacted with regard to this Request. No other SMH staff, administrators, or board members shall be contacted. Failure to comply could result in immediate disqualification at the discretion of the Director of Architecture and Construction.

- The selection committee will meet in a public meeting on April 26, 2019 in the Waldemere Auditorium, located at 1700 S. Tamiami Trail, Sarasota, FL, 34239, level one, Waldemere Tower, 8:00 a.m. – 11:00 a.m. to discuss and announce the top three ranked firms.
- The Hospital will subsequently engage in contract negotiations. All interested parties are invited to attend.
