NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 42 No. 114, June 13, 2016 issue of the Florida Administrative Register.
The change is in response to written comments submitted by the staff of the Joint Administrative Procedures Committee and by a discussion and vote by the Board at its January 27, 2017, meeting. The rule shall now read as follows:
64B2-18.002 Application for Certification as a Chiropractic Physician’s Assistant.

The following changes have been made to incorporated form DH-MQA 1148 (Rev. 07/16):
Language regarding a sworn statement or affidavit from the applicant has been added to the application.
Page 1, section 2: the formatting and lettering of the paragraphs have been corrected.
Page 2, section 3: the nonrefundable fee language has been removed.
Page 2, section 5: the formatting of the Department of Health’s address has been corrected.
Page 3, section 1: the nonrefundable fee language has been removed.
Page 3, section 7: the prevention of medical errors course requirement has been removed.
Page 6, section 4: the request for the applicant’s place of birth has been removed.
Page 7: the formatting of the “Important Notice” has been corrected.
