    Community Care for the Elderly Request for Proposal

    Competitive sealed proposals will be received by the Area Agency on Aging for North Florida until 4:00 p.m. on May 20, 2016 for the designation of Community Care for the Elderly Lead Agencies. The Community Care for the Elderly Lead Agency designation includes the provision of an array of home and community based services to frail older persons. A Community Care for the Elderly Lead Agency will be designated for each county in Planning and Service Area 2. These counties are: Bay, Calhoun, Franklin, Gadsden, Gulf, Holmes, Jackson, Jefferson, Leon (also includes Local Service Program for expanded Adult Day Health Care and Case Management), Liberty, Madison, Taylor Wakulla, and Washington. The Community Care for the Elderly program is administered pursuant to provisions of Chapter 430, F.S. The proposal package and application instructions may be obtained are available on the Vendor Bid System at http://myflorida.com/apps/vbs/vbs_www.main_menu and from the Area Agency on Aging for North Florida on April 20, 2016 or thereafter at the correspondence address listed below. The Area Agency on Aging for North Florida reserves the right to reject any and all proposals.

    Every entity that intends to submit a proposal must deliver the required Notice of Intent Form (Appendix III) to the Area Agency on Aging for North Florida, Inc. at the correspondence address provided below by 4:00 p.m. on Friday, May 1, 2015. In the event that less than two Notices of Intent are received per county, the Area Agency on Aging for North Florida will institute the Exceptional Purchase (as defined in Section 287.012, Florida Statutes) provisions provided for by Chapter 287, Florida Statutes for a single source contract. In which case no response to Request for Proposals shall be needed, and the Agency will proceed under the single source provisions of Section 287.057, Florida Statutes.

    Any entity that protests the intended decision pertaining to the Exceptional Purchase shall be required to post, at the time of filing a formal written protest, a bond in the amount equal to 1 percent of the estimated contract amount for the Exceptional Purchase pursuant to Section 287.042, Florida Statutes.

    A Bidders Conference will be conducted concerning this Request for Proposals at 10:00 a.m. on April 24, 2015.
