    64E-3.001Fees

    PURPOSE AND EFFECT: The Department determined that this rule amendment is necessary to promulgate a rule form and possible rule language that solicits the information from a qualifying veteran or the veteran’s spouse to obtain an initial application fee waiver for an application for certification under Chapter 468 Part IV, Florida Statutes.

    SUMMARY: Promulgates rule form required to obtain information from veterans and veteran’s spouses for an initial application fee waiver for certification for licensure under Chapter 468 Part IV, Florida Statutes.

    SUMMARY OF STATEMENT OF ESTIMATED REGULATORY COSTS AND LEGISLATIVE RATIFICATION: The Agency has determined that this will not have an adverse impact on small business or likely increase directly or indirectly regulatory costs in excess of $200,000 in the aggregate within one year after the implementation of the rule. A SERC has not been prepared by the Agency.

    The Agency has determined that the proposed rule is not expected to require legislative ratification based on the statement of estimated regulatory costs or if no SERC is required, the information expressly relied upon and described herein: This rule amendment will not have an adverse economic impact. Based on 2014 Department data, the amendment will provide a form that can be used to waive the initial application fees currently in rule for veterans or veteran’s spouses who apply for certification under Chapter 468 part IV Florida Statutes. Fees currently in rule for other applicants will not change. Therefore, this rulemaking will not have an adverse impact or regulatory costs in excess of $1 million within five years as established in Section 120.541(2)(a), F.S.

    Any person who wishes to provide information regarding a statement of estimated regulatory costs, or provide a proposal for a lower cost regulatory alternative must do so in writing within 21 days of this notice.

    The following fees are prescribed by the department:

    (1) through (8) No Change.

    (9) All persons seeking a veteran’s or veteran’s spouse fee waiver pursuant to Section 468.304(1), F.S., for the initial application fee shall submit to the Department a completed “Military Veteran or Spouse Fee Waiver Request for Chapter 468 part IV Certification,” Form DH 9002- EPCS-06/2015, which is herein incorporated by reference and is available from the internet at http//www.floridahealth.gov/radiation, and at ____________.
