NOTICE IS HEREBY GIVEN that the Board of Dentistry has received the petition for declaratory statement from Cynthia A. Mikos, Esquire, on behalf of Tatiana Rocha Oliveira, D.D.S. filed on April 27, 2017. The petition seeks the agency’s opinion as to the applicability of Section 466.006(3)(b), F.S., and Rule 64B5-2.0146, F.A.C. as they apply to the petitioner.

The Petitioner seeks a Declaratory Statement from the Board with regard to Section 466.006(3)(b), Florida Statutes, and Rule 64B5-2.0146, Florida Administrative Code, as to whether the Petitioner has met all requirements of completion of a supplemental general dentistry program based on the particular set of circumstances presented in the Petition. Except for good cause shown, motions for leave to intervene must be filed within 21 days after the publication of this notice.
