Notice to Vacate Emergency Order
On April 28, 2014, the State Surgeon General, issued an Order Vacating Order of Emergency Suspension of License with regard to the license of Raymond Failer, D.O., License Number: OS 1920. The Department orders that the Emergency Suspension of License be vacated.
