    PURPOSE AND EFFECT: The purpose of the proposed amendments to Rule 12E-1.030, F.A.C. (Administrative Establishment of Child Support Obligations), is to incorporate by reference changes to forms CS-ES96, CS-OA140R, and CS-OA178 the Department uses in administrative proceedings.

    The effect of the amendments is to revise terminology in forms the Department uses in administrative proceedings.

    The purpose of the proposed amendments to Rule 12E-1.036, F.A.C. (Administrative Establishment of Paternity and Support Obligations), is to incorporate by reference the Family Law Financial Affidavit (CS-PO30) used to determine annual income when establishing child support obligations and incorporate by reference changes to forms CS-PO31, CS-OA01, CS-OA12, CS-OP50, CS-OA40, and CS-OX40 the Department uses in administrative proceedings.

    The effect of the amendments is to incorporate by reference the Family Law Financial Affidavit and revise terminology in forms the Department uses in administrative proceedings.

    The purpose of the proposed amendments to Rule 12E-1.039, F.A.C. (Request for Services), is to incorporate by reference changes to the Paternity Declaration, form CS-PO34, the Department uses in administrative proceedings.

    The effect of the amendments is to revise terminology in a form the Department uses to collect information necessary to process requests for services.
