    NOTICE OF CHANGE

    Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 44 No. 76, April 18, 2018 issue of the Florida Administrative Register.

    69O-203.021 Standards for Fingerprint Cards for New Applicants and Acquisition Applications.

    (1) Fingerprints shall be submitted only on a card, for which instructions are adopted in subsection 69O-203.100(9), F.A.C., provided by the Office of Insurance. Others will not be accepted.

    (2) The Office has adopted Form OIR-C1-938 (rev. 04/18 5/13), incorporated by reference in Rule 69O-203.100, F.A.C., which provides instructions on fingerprinting how fingerprint cards are to be completed.

     

    69O-203.202 Standards for Discount Medical Plans.

    (1) No change.

    (a) Name and address of the DPO;

    (b) through (i) No change.

    (j) Provisions for adding new family members; and

    (k) All plan contracts and application forms shall have a unique form number in the lower left hand corner; and

    (k)(l) Member complaint procedure.
