    64J-1.011Involuntary Inactive Certification

    NOTICE IS HEREBY GIVEN that on May 12, 2014, the Department of Health, Bureau of Emergency Medical Oversight, EMS Program, received a petition for Variance or Wavier from Florida Administrative Code, subsection 64J-1.011(3), for an individual that failed to renew his certification within 180 days of the expiration date, December 1, 2012. Mr. George C. Beutell is requesting a variance or wavier from subsection 64J-1.011(3), F.A.C., “ An application for recertification received by the department more than 180 days after the expiration date of the certification shall be denied. Such certificate holder is ineligible for recertification and must meet the requirements for initial certification”.

    Interested parties may submit comments within 14 days of publication of this notice.
