NOTICE IS HEREBY GIVEN THAT on April 30, 2008, the Department of Health, received a petition for subparagraph 64E-13.004(6)(a)1., Florida Administrative Code, from Spirit Christian Academy, Inc., 1154 Havendale Blvd. N.W., Winter Haven, Florida.
