NOTICE IS HEREBY GIVEN that on May 18, 2023, the Board of Opticianry, received a petition for variance and waiver filed by Katelyn Jones. Petitioner seeks a variance or waiver of Rule 64B12-16.004, F.A.C., which requires the apprentice to submit Form DH-MQA 1063 within 30 days if an apprentice terminates a sponsorship with a sponsor upon completion of the program hours, or the sponsor is no longer providing training to an apprentice.
