The Agency for Healthcare Administration has received an application for services exemption from Florida Hospital Ormond Memorial and Florida Hospital Oceanside, 875 Sterthaus Avenue, Ormond Beach, FL 32174 and 264 South Atlantic Avenue, FL 32176 pursuant to Section 395.1041(3), Florida Statutes, and Rule 59A-3.255, Florida Administrative Code. The services category which the exemptions are requested is: Neurosurgery. Comments received within 15 days of publication will be considered by the Agency prior to making a determination exemption status.
