PURPOSE AND EFFECT: To update the following forms: Form VSP-03 Course Provider Approval Application; Form VSP-04 Application for Currently Approved Online Course Provider, and Form VSP-05 Course Renewal Provider Application. Having received stakeholder input, the applications will be revised by rephrasing and deleting questions and prompts for clarity.
