NOTICE OF CHANGE

Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 47 No. 68, April 8, 2021 issue of the Florida Administrative Register.

The changes are in response to written comments submitted by the staff of the Joint Administrative Procedures Committee in a letter dated April 19, 2021. The changes are as follows:

Incorporated by reference Form DBPR CILB 2, Application for Registered Contractor as an Individual, the Department includes changes on page 2, Section II- Applicant Licensure Category: The “Note” in section II has been changed to include “Tank Lining Applicator applicants”.

Incorporated by reference Form DBPR CILB 3, Application for Registered Contractor Qualifying a Business, the Department includes changes on page 2, Section II- Applicant Licensure Category: The “Note” in section II has been changed to include “Tank Lining Applicator applicants”.

Incorporated by reference Form DBPR CILB 4, Application for Registered Contractor Qualifying an Additional Business Entity Under a New License Category, the Department includes changes on page 2, Section II- Applicant Licensure Category: The “Note” in section II has been changed to include “Tank Lining Applicator applicants”.

The following Construction Industry Departmental forms can be obtained at www.myfloridalicense.com/dbpr/ or by mail from the Department of Business and Professional Regulation, 2601 Blair Stone Road, Tallahassee, FL 32399-0790, [phone]. After July 16, 2012, the Department will only accept these forms for application purposes.

(1) through (3)(h) No change.

(j) Persons applying to become licensed as a Building Demolition Specialty Contractor shall use the following forms:

1. Form DBPR CILB 5-AD Application for Certified Building Demolition Specialty Contractor as an Individual, effective XXXXX, is adopted and incorporated by reference,
