64B12-16.003: Apprenticeship Requirements and Training Program

NOTICE OF CHANGE

Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 46 No. 49, March 11, 2020 issue of the Florida Administrative Register.

The Board received written comments from the Joint Administrative Procedures Committee on Rule 64B12-16.003.  Additionally, the Board made changes to the proposed rule and form set forth in the rule to reflect a new form title and revision date as (Apprentice Optician Apprentice Application for Registration Form DH-MQA 1180, revised 03/20 11/19).  Further, the word “affirm” located on pages 9 and 11 of form DH-MQA 1180 have been changed to the word “state”.
