NOTICE OF CHANGE

Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 48 No. 10, January 14, 2022 issue of the Florida Administrative Register.

A previous Notice of Change was published on May 23, 2022, in Vol. 48, No. 100. The changes are in response to written comments submitted by the staff of the Joint Administrative Procedures Committee.  The changes are as follows:

 

61G15-20.0017 Application for Retired Status.

(1) through (4) No change.

The following changes have been made to incorporated form of FBPE/005 Application for Retired Status:

 

• Page 1 of 2, has been removed and amended to one page only due to duplication.
• The form has been amended to read the correct board address and contact information to reflect Rule 61G15-20.0017 text.
• The form has been updated and amended with Revision Date from (12/2021) (06-01) to read as (Rev. 04/2022)
• The statement “I further verify that I have no pending complaints against my Florida license or….” has been removed.
• The form has added an email address (board.fbpe.org) to send completed forms to the board.
• The rule number has been added to the bottom of the form.
