Notification of Approval and Denial of Provisional Trauma Center Applications
The Department of Health (Department) gives notice that Memorial Hospital (TSA 5) has successfully completed the provisional review phase of the Level II trauma center approval process as specified in Section 395.4025(2)(c), Florida Statutes and paragraph 64J-2.012(1)(d), Florida Administrative Code.
The Department further gives notice that Northside Hospital’s Level II application (TSA 9) and Wolfson Children’s Hospital’s Pediatric application(TSA 5) did not successfully complete the provisional review as specified in section 395.4025(2)(c), Florida Statutes and paragraph 64J-2.012(1)(d), Florida Administrative Code and were denied by the Department.
Any hospital that wishes to protest one or more of these decisions may seek review pursuant to sections 120.569 and 120.57, Florida Statutes, by filing a petition with the Agency Clerk by mail to the Agency Clerk, Department of Health, 4052 Bald Cypress Way, BIN #A-02, Tallahassee, FL 32399-1703, via hand delivery to the Agency Clerk, Department of Health, 2585 Merchants Row Blvd., Prather Building, Suite 110, Tallahassee, FL, or via facsimile transmission to the Agency Clerk, Department of Health, (850)413-8743. Such petition must be filed in compliance with Florida Administrative Code, Rules 28-106.201 or 28-106.301 as applicable. Mediation is not available. A petition for an administrative hearing must be received by the Department on or before the 21st day after your receipt of this notice.
