NOTICE IS HEREBY GIVEN that Agency for Health Care Administration has received the petition for declaratory statement from Advinity, LLC. The petition seeks the agency’s opinion as to the applicability of 400.980(1)(b) and 400.980(2), Florida Statutes (2017), as it applies to the petitioner.

Please note this is not a new Notice of Petition for Declaratory Statement. This Notice of Petition is an amendment to the Notice of Petition published on May 4, 2018, Volume 44/88. The issue needing determination is whether or not its organization, Advinity, LLC, is considered a “Health Care Services Pool” pursuant to 400.980, F.S.
