NOTICE IS HEREBY GIVEN that the Department of Financial Services, Division of Workers’ Compensation (the “Department”) has received the petition for declaratory statement from Sainaam Realty & Investment, Inc. (the “Petitioner”) on March 12, 2015. The petition seeks the agency’s opinion as to the applicability of workers’ compensation compliance violation as it applies to the petitioner.

The Petition seeks the Department’s opinion as to why the Petitioner was investigated for a workers’ compensation compliance violation, why Petitioner was issued an order of penalty assessment, the number of employees for whom Petitioner failed to secure the payment of workers’ compensation, and how the penalty was calculated.
