Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 48 No. 62, March 30, 2022 issue of the Florida Administrative Register.

Form DFS-H2-6365 is also amended as follows: On page 18, step 6, the affirmation statement that included swearing under the penalties or perjury was removed. The language on page 18, paragraph 3, was amended read: “I have read the foregoing application and the facts stated in it are true to the best of my knowledge and belief. I have not withheld any information that would in any way affect my qualifications. I understand that misrepresentation of any fact required to be disclosed through this application may result in the denial of my application and the revocation of my insurance license(s).” Technical changes were made to remove references to a previous form number and effective date within screenshots of the online form on pages 5, 12, 14, 15, 16, and 18.
