NOTICE TO PROFESSIONAL CONSULTANTS
STATE OF FLORIDA
DEPARTMENT OF CORRECTIONS
PUBLIC ANNOUNCEMENT FOR PROFESSIONAL ARCHITECTURAL SERVICES
The State of Florida, Department of Corrections, Bureau of Support Service, Central Procurement Office, announces that Professional Services are required from Architectural firms to provide Continuing Architectural services for projects in Region I, Region II, Region III, and Region IV. Applications from qualified firms are to be sent to the attention of Paul Cowan, Bureau of Support Services, Department of Corrections, 501 South Calhoun Street, Tallahassee, Florida 32399-2500, (850) 717-3700. The selected firm may be required to perform its services on an accelerated schedule.
INSTRUCTIONS
Any firm desiring to provide professional services for this project shall apply for consideration with a letter of application, indicating within the body of the letter, your firm’s specific abilities respective to the particular project’s requirements, office location(s) from which the firm will be doing the work, and attach current copies of the following:
1. A completed copy of the Department of Corrections’ current “Professional Qualifications Supplement” (PQS) (dated September 2004 and revised March 2015) with current data. The PQS is posted as a related document for printing on the Vendor Bid System under this solicitation at the following website address: http://myflorida.com/apps/vbs/vbs_www.search.criteria_form.
2. A reproduction of the firm’s current Florida State Board License with the appropriate board governing the discipline offered. If the firm is a corporation, it must be properly chartered with the Department of State to operate in Florida. Include a copy of the charter and current certification that all fees and filings are current.
3. Attach a current copy of the SF330 for the office(s) that will be performing the work. This form may be found at the following website address: http://www.gsa.gov.
4. Evidence of professional liability insurance in the amount of at least $1,000,000.
5. Describe scope of work to be assigned to any sub-consultant and include a complete SF330 for each sub-consultant.
6. Names, specific qualifications, and professional memberships of person(s) to be assigned to this project and their respective roles (do not include resumes).
7. Names, addresses, and phone numbers of at least five (5) other agencies for whom similar services have been performed within the last five (5) years and the date, project name, brief project description, firm’s project manager and specific services provided in each case.
8. If desired by the firm, additional examples of similar projects completed by persons to be assigned to this project, references (none from current or former Department of Corrections personnel) and any other supporting information.
Submit one original letter of interest and one original with three (3) copies of the required qualification data. The required data shall be submitted in the order listed above. Faxed copies will not be accepted. Applications that do not comply with the instructions set forth above may be considered improper and be disqualified. The plans and specifications developed by the firm awarded this project are subject to reuse, in accordance with the requirements of Section 287.055, Florida Statutes (F.S.), (“Consultants’ Competitive Negotiation Act”).
The awarded firm shall not knowingly engage in this project, on a full time, part time, or other basis during the period of this Contract, any current or former employee of the Department, where such employment conflicts with Section 112.3185, F.S.
Respondents will be ranked by a Competitive Selection Committee based on the information received in response to this Request for Qualifications (RFQ). Interviews of all ranked respondents may or may not be required, at the discretion of the Committee. If interviews/presentations are required, they will be held during the weeks of July 11, 2016, and July 18, 2016. July 25, 2016, and August 1, 2016. Under the authority delegated to the Secretary of the Department of Corrections by Section 287.055, F.S., sitting as the head of the Department of Corrections, Professional Services Contracts will be awarded to one firm in each of the four regions following the recommendations of the Competitive Selection Committee, and in accordance with the statutory negotiation procedures. All firms responding to this RFQ will be notified by a single posting through the following website address: http://myflorida.com/apps/vbs/vbs_www.search.criteria_form. It is the firm’s responsibility to check the website for updated information.

Response Date: June 17, 2016 June 30, 2016 by 5:00 P.M., Eastern Time. Late submittals will not be opened or considered.
PROJECT: Provide Continuing Architectural Services for projects in which basic construction cost, for each project, does not exceed $2,000,000 or for a study activity when the fee for professional services does not exceed $200,000.
CONTRACT TERM: Services are for projects authorized within a two (2) year period with an option to renew for one (1) additional two (2) year period.
EXPERIENCE: Candidate firms need to provide information regarding their experience and expertise in Architectural design; structural design of institutional buildings, small buildings, additions to buildings, reroofing, and renovation projects for correctional institution application.
LOCATION: Most of the work will be related to working on prototype designs for projects for Region I, Region II, Region III, and Region IV; therefore, the following locations are designated as the project sites for the purposes of the selection process:
Region I Central Office – Located at 501 South Calhoun Street in Tallahassee, Florida
Region II Reception and Medical Center (RMC) – Located at 7765 S. CR 231 Lake Butler, Florida
Region III Polk CI - Located at Located at 10800 Evans Road in Polk City, Florida
Region IV South Florida Reception Center (SFRC) - Located at 14000 NW 41st Street in Doral, Florida

PROFESSIONAL QUALIFICATIONS SUPPLEMENT (PQS)

PURPOSE:
The Professional Qualifications Supplement (PQS) is to provide information regarding the qualifications of interested firms to perform Architectural Services on projects being administered by the Florida Department of Corrections (FDC) in accordance with the requirements of Section 287.055, F.S., "Consultants' Competitive Negotiation Act."
INSTRUCTIONS:
Type accurately; instruction numbers correspond to the numbers on the form; additional pages corresponding to any of the numbered items may be attached.
For the APPLICANT ONLY: attach a copy of the current Florida Department of Professional Regulation License(s) with the appropriate Board(s) for each of the license numbers listed in Number 3.
1.Complete the Project Number or Area Designation, and Project Name as it appears in the Public Announcement for Professional Services advertised on the MyFlorida.com, Vendor Bid System Website Address: http://myflorida.com/apps/vbs/vbs_www.search.criteria_form
2.Provide the complete Firm Name of the applicant as it appears on the Corporate Seal (if your firm is incorporated), the Federal Identification Number (FEID), address, telephone and fax number of your office, and contact name. If your firm has two or more office locations, then list the address of the office from which the project(s) will be managed throughout the design and construction phases.
3.For the APPLICANT and the PROPOSED CONSULTANTS, place an X by the service(s) to be provided on the project. Enter the firm's appropriate Department of Professional Regulation license number. *If the firm is a corporation, enter the Florida Corporate Charter Number as issued by the Division of Corporations, Department of State.
*Use the license number in accordance with the name of the firm as presented on the firm's letterhead. Use individual license numbers only when applicable.
4.List, for the APPLICANT firm only, each project currently under contract including contracts as a consultant to another firm. For projects on hold for twelve (12) months or longer, enter the amount of fees remaining (unearned) in the Fees on Hold column. Attach a letter from the owner stating how long the project has been on hold and how much longer it is expected to remain on hold.
For all of the other projects, enter the fees (unearned) in the Fees Remaining column. Fees for additional services are to be included. Do not include fees to consultants. Projects shown as “On Hold,” but for which a letter from the owner is not submitted, will be added to the Fees Remaining column. Divide the sum of the Fees Remaining column by the number of licensed personnel, graduate design professionals, lead draftsmen, specification writers and designers, but do not include draftsmen and CADD operators in the number of technical staff. Indicate the total number of current projects (including projects on hold).
5a.For the APPLICANT: list all active Department of Corrections commissions for ALL offices. Indicate total commissions on contract(s) executed for the periods noted. If none, indicate zero.
5b.Indicate the total number of Florida licensed personnel employed by your firm (this data is to be entered whether or not you have or have had contracts with FDC). Indicate the fee per person (licensed personnel) by dividing the total Fee Considered in 5a by the number of licensed personnel. If 5a is zero enter zero.
6.List projects comparable to this specific project and related experience accomplished by the applicant. Indicate name of project, completion date, its location, construction cost and phase of project at this time, if applicable.
7.Designate the proposed project team key personnel for the applicant and consultants. For each individual listed, show their discipline(s) of licensure/training, their city of residence and their primary office work location.
8.Sign and date the form; type the name and title of the individual signing.
NOTE:Public Records: To the extent that information is utilized in the performance of the resulting contract or generated as a result of it, and to the extent that information meets the definition of “public record” as defined in subsection 119.011(1), F.S., said information is hereby declared to be and is hereby recognized by the parties to be a public record and absent a provision of law or administrative rule or regulation requiring otherwise, shall be made available for inspection and copying by any person upon request as provided in Art. I, Sec. 24, Fla. Constitution and Chapter 119, F.S. It is expressly understood that any state contractor’s refusal to comply with these provisions of law shall constitute an immediate breach of the contract resulting from this selection entitling the department to unilaterally terminate the contract. The successful respondent will be required to notify the department of any requests made for public records.
Unless a greater retention period is required by state or federal law, all documents pertaining to the program contemplated by this solicitation shall be retained by the successful respondent for a period of ten years after the termination of the resulting contract or longer as may be required by any renewal or extension of the contract. During this period, the successful respondent shall provide any documents requested by the Department in its standard word processing format (currently Microsoft Office 2007). If this standard should change, the successful vendor shall adopt the new standard at no cost to the department. Data files will be provided in a format directed by the department.
The successful respondent agrees to maintain the confidentiality of all records required by law or administrative rule to be protected from disclosure. The successful respondent further agrees to hold the department harmless from any claim or damage including reasonable attorney’s fees and costs or from any fine or penalty imposed as a result of failure to comply with the public records law or an improper disclosure of confidential information and promises to defend the department against the same at its expense.
Disclosure of Reply Contents: All documentation produced as part of this selection shall become the exclusive property of the Department and may not be removed by the Respondent or its agents. All replies shall become the property of the Department and shall not be returned to Respondent. The Department shall have the right to use any or all ideas or adaptations of the ideas presented in any reply. Selection or rejection of a reply shall not affect this right.
Confidential, Proprietary, Or Trade Secret Material: The Department takes its public records responsibilities as provided under chapter 119, Florida Statutes and Article I, Section 24 of the Florida Constitution, very seriously. If Respondent considers any portion of the documents, data or records submitted in response to this solicitation to be confidential, trade secret or otherwise not subject to disclosure pursuant to chapter 119, Florida Statutes, the Florida Constitution or other authority, the Respondent must also simultaneously provide the Department with a separate redacted copy of its response and briefly describe in writing the grounds for claiming exemption from the public records law, including the specific statutory citation for such exemption. This redacted copy shall contain the Department’s selection name, number, and the name of the respondent on the cover, and shall be clearly titled “Redacted Copy”. The Redacted Copy shall be provided to the Department at the same time Respondent submits its response to the selection and must only exclude or redact those exact portions which are claimed confidential, proprietary, or trade secret. The Respondent shall be responsible for defending its determination that the redacted portions of its response are confidential, trade secret or otherwise not subject to disclosure. Further, Respondent shall protect, defend, and indemnify the Department for any and all claims arising from or relating to Respondent’s determination that the redacted portions of its response are confidential, proprietary, trade secret or otherwise not subject to disclosure. If Respondent fails to submit a Redacted Copy with its response, the Department is authorized to produce the entire documents, data, or records submitted by Respondent in answer to a public records request for these records. In no event shall the Department, or any of its employees or agents, be liable for disclosing, or otherwise failing to protect, the confidentiality of information submitted in response to this selection.
Identical Tie Responses: When evaluating bids/proposals/responses to selections, if the Department receives identical pricing or scoring from multiple Respondents, the Department shall determine the order of award using the criteria set forth in Rule 60A-1.011, FA.C. and Chapter 295.187, F.S.
