NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 43 No. 91, May 10, 2017 issue of the Florida Administrative Register.

(1) through (5) No change.
(6) Each licensed midwife, temporary certificate holding midwife, and midwife supervising a student midwife in assisting in childbirth that occurs in an out-of-hospital setting, shall file an annual report no later than July 31 for the prior fiscal year on Form DH-MQA 5011, Annual Report of Midwifery Practice (06/2017 04/2017), incorporated by reference and available at https://www.flrules.org/Gateway/reference.asp?No=Ref-_____06545.
(7) No change.
Form DH-MQA 5011 was changed to add instructions to assist reporting midwives.
