    6E-2.004Standards and Procedures for Licensure

    PURPOSE AND EFFECT: To consider amendments to Institutional Licensure Standards. These will include at least the following five areas:

    1) Amendments to Form 804, entitled Placement and Retention Reporting for Institutionally Accredited Institutions Programmatic Reporting by adding a comment field or another mechanism for institutions to provide information when rates do not meet standards and to request submission of improvement plans;

    2) Amendments to Form 604 entitled Selected Financial Data to modify the signature requirements;

    3) Amendments requiring a form to document any clinical experience, internship, externship, or practicum, and the elements of any such form;

    4) Amendments increasing the standards and time frames before institutions may offer college level instruction; and

    5) Amendments to the standard on advertising for discounts and scholarships.
