NOTICE IS HEREBY GIVEN that on June 21, 2013, the Board of Orthotists and Prosthetists received a petition for Variance or Waiver from Wayne Lawall. Petitioner is seeking a permanent variance or waiver of paragraph 64B14-4.003(1)(d), F.A.C. which requires those seeking to obtain a license as an Orthotists or Prosthetists in the State of Florida and complete an internship for certification must provide a patient log signed by his qualified supervisor(s) and the number of hours worked.
