Project Description: Add 10 comprehensive medical rehabilitation beds

Facility/Project: Park Meadows Health and Rehabilitation Center

Applicant: The Oaks NH, LLC

Project Description: Add 30 community nursing home beds to the Park Meadows Health and Rehabilitation Center replacement facility approved via CON #10350

Proposed Project Cost: $2,220,000
