    NOTICE IS HEREBY GIVEN that the Agency for Health Care Administration has received the petition for declaratory statement from Anthony Edmands by and through the Law Offices of Peter A. Lewis, P.L. amending the petition filed on May 22, 2014.The petition seeks the agency's opinion as to the applicability of Section 111, F. of the Florida, Title XIX, Long Term Care Reimbursement Plan, which has been incorporated by reference in Rule 59G-6.010, Florida Administrative Code as it applies to the petitioner.

    The Petitioner is requesting that the Agency issue a Declaratory Statement advising whether the Petitioner's applicant entities are "related" parties to the current licensed operators/providers.
