    NOTICE OF CHANGE

    Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 39, No. 106, May 31, 2013 issue of the Florida Administrative Register.

    (a) through (c) No change.

    (d) Persons applying for a non-resident temporary practice permit shall use Form DBPR FREAB 13, Application for Non-resident Temporary Practice Permit, effective July 2013 February 2013, is adopted and incorporated by reference, http://www.flrules.org/Gateway/reference.asp?No=Ref-03051 http://www.flrules.org/Gateway/reference.asp?No=Ref-01986.

    Form DBPR FREAB 13, “Application for Non-resident Temporary Practice Permit,” effective February 2013 is amended as follows:

     

    On page 1: The citation for the definition of a “federally related transaction” has been updated to Section 475.611(1)(p).

    The Instructions section has been updated to include notices that:

    “Email addresses provided to the Board are public record”, and;

    “Providing contact information and business contact information is voluntary.”

    The date for the form is updated in the footer to read “2013 July.”
