NOTICE OF CHANGE

Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 43 No. 92, May 11, 2017 issue of the Florida Administrative Register.

The change is in response to written comments submitted by the staff of the Joint Administrative Procedures Committee and from a vote by the Board at a duly noticed public meeting held on June 2, 2017.  The rule shall now read as follows:

64B3-5.008 Public Health Laboratory Personnel.

(1) through (3) No change.

(4) All applicants for licensure as a Public Health Laboratory Scientist shall apply to the Department on Form # DH-MQA 3001 (06/17) (07/16) “Application for Public Health Laboratory Scientist” which is incorporated by reference herein, copies of which can be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-      or the Board office at 4052 Bald Cypress Way, Bin #C07, Tallahassee, Florida 32399-3257 or from its website at http://floridasclinicallabs.gov/applications/app-public-health-lab-clp.pdf.

The following changes have been made to incorporated form DH-MQA 3001:

The revision date of the form has been changed to 06/17.

On pages 8-9, language regarding a sworn statement under oath from the applicant has been added to the application.
